DÍA MUNDIAL DE LINFEDEMA - MANIFIESTO DE LINFEDEMA – 6 de marzo de 2020 – Mes de Sensibilización -




Para resaltar las necesidades de los/as pacientes en el marco de la sanidad pública, desde la Federación Española de Asociaciones de Linfedema y Lipedema (FEDEAL) de la que forma parte AGL, se elaboró el siguiente documento y lista de puntos que están enmarcados en el MANIFIESTO NACIONAL.



MANIFIESTO FEDEAL POR EL DÍA MUNDIAL DEL LINFEDEMA

En todas las Comunidades del Estado Español existen personas afectadas de Linfedema y Lipedema, y en base a nuestra experiencia sabemos que no están diagnosticadas o lo son de forma tardía, muchas no reciben una información correcta sobre la enfermedad, hay largas listas de espera o problemas para acceder a un tratamiento intensivo que incluya el necesario mantenimiento, y muchos pacientes se ven forzados a costearse su propia atención médica.

Por todo ello:



1. Defendemos una Sanidad Pública, de acceso universal, tal y como queda reflejado en nuestra Constitución. Nuestro objetivo es recibir una calidad asistencial de los servicios de los que somos usuari@s direct@s.



2. Queremos manifestar la necesidad de una cohesión territorial en cuanto a prestaciones y tratamiento a través de la Sanidad Pública, sin discriminación o exclusión por razones de residencia, en relación con métodos de diagnóstico y a utilización de tratamientos de calidad, incluso los de reciente aparición.



3. Queremos proponer la implantación de Unidades de Linfedema y Lipedema en Hospitales, dotados de equipos multidisciplinares que puedan proporcionar una atención sanitaria de calidad y ofrecer una atención integral en todos los aspectos médicos y psicosociales dentro de la sanidad pública.



4. Consideramos fundamental conocer la prevalencia y la incidencia del Linfedema Primario y Secundario y Lipedema para que puedan así destinarse los recursos necesarios al tratamiento de esta enfermedad. Queremos impulsar estudios epidemiológicos que aborden problemas de salud de las personas afectadas por linfedema y lipedema.



5. Se ha de promover y apoyar la investigación acerca del Linfedema, Lipedema y Flebedema, así como de todos sus posibles tratamientos para que se incluyan en los Planes Nacionales de Salud.



6. Pedimos que se revisen algunas prácticas actuales que se utilizan como tratamientos que suponen un peligro o empeoramiento para la salud, como son la prescripción indiscriminada de determinados fármacos o la mala utilización de los sistemas de presoterapia, entre otros.



7. Entendemos que la edad no debe ser motivo de exclusión en la valoración y tratamiento de la enfermedad ya que al tratarse de patologías crónicas puede manifestarse en cualquier etapa de la vida.



8. Solicitamos que se instaure programas de detección precoz del Linfedema, Lipedema y Flebedema, para que l@s y los profesionales de la salud puedan derivar a la persona afectada al servicio correspondiente: Angiología y Cirugía Vascular, Medicina Interna, Dermatología y Servicio de Medicina Física y Rehabilitación, etc.



9. Debe existir una mayor información tanto para l@s profesionales de la salud como para el/la paciente, para poder acceder al tratamiento idóneo lo antes posible, evitando el empeoramiento de la enfermedad, que conduce a un estado irreversible y a un mayor grado de invalidez, lo que puede repercutir negativamente en el acceso al mundo escolar, laboral y en la integración social.



10. Solicitamos que el Ministerio de Sanidad y Ciencia elimine de su listado como posible pseudoterapia en estudio el Drenaje Linfático Manual, puesto que el Consejo General de Colegios de Fisioterapeutas de España reconoce dicha técnica como indispensable para el tratamiento de una de la secuelas más importantetes en la mastectomías, como es el linfedema. Asimismo solicitamos que el Sistema Público Sanitario financie las terapias de mantenimiento con drenaje linfático manual adecuada a cada paciente, ya que actualmente la está costeando dl/la paciente.



11. Consideramos preciso incluir en la cartera de prestaciones de las Consejerías de Salud, las prendas ortopédicas adecuadas a las necesidades de cada paciente.



12. Pedimos que se tengan en cuenta las limitaciones que produce el Linfedema y el Lipedema en el momento de realizar valoraciones de Incapacidad Laboral, Discapacidad y Dependencia.



13. Queremos ser un órgano interlocutor válido ante las Instituciones Sanitarias en defensa del paciente, estableciendo relaciones de formación, información, coordinación y cooperación con las instituciones y con asociaciones afines tanto nacionales como internacionales.

AVISO IMPORTANTE 

El Linfedema es una enfermedad crónica y progresiva debido a una discapacidad orgánica. El tratamiento clínico del linfedema no es ni un tratamiento cosmético ni estético. El tratamiento del linfedema es para controlar su avance y aliviar los síntomas relacionados con la disfunción del sistema circulatorio linfático. Para prevenir y evitar problemas graves asociados al tratamiento, como el posible desplazamiento del edema a zonas previamente no afectadas cuando se aplica la terapia de compresión para la reducción del volumen, los pacientes deben recurrir a terapeutas altamente especializados con experiencia. La cualificación profesional y la instrucción impartida a distancia en línea no son lo mismo que la instrucción práctica en directo en la formación clínica y la certificación de los terapeutas del linfedema, similar a todos los demás programas de formación en rehabilitación, medicina y cirugía, y especialmente para desarrollar las habilidades manuales necesarias para tratar una enfermedad tan compleja como el linfedema.

Los pacientes deben igualmente tener un especial cuidado a la hora de elegir un "Centro Multidisciplinar de Referencia de Linfedema", ya que no todos los países o centros de referencia proporcionan las mismas opciones de tratamiento. La mejor elección es un centro de referencia que aplica la Terapia Descongestiva Compleja (TDC), que está reconocida como el tratamiento "Estándar de Excelencia" para el linfedema.

To highlight the needs of patients within the framework of public health, the Spanish Federation of Lymphoedema Associations (FEDEAL), of which AGL is a member, drew up the following document and list of points that are framed in the NATIONAL MANIFESTO.

In all the Autonomous Regions of the Spanish State there are people affected by Lymphedema and Lipedema, and based on our experience we know that they are not diagnosed or are diagnosed late, many do not receive correct information about the disease, and there are long waiting lists or problems in accessing intensive treatment including the necessary maintenance therapy, and many patients are forced to pay for their own medical care.

For the above reasons:


1. We defend public health care, with universal access, as reflected in our Constitution. Our objective is to receive quality care from the health services of which we are direct users.



2. We wish to express the need for territorial cohesion in terms of benefits and treatment through public health care, without discrimination or exclusion on the grounds of residence, concerning diagnostic methods and the use of quality treatments, including those of recent emergence.



3. We would like to propose the implementation of Lymphoedema and Lipedema Units in Hospitals, equipped with multidisciplinary teams that can provide quality health care and offer comprehensive care in all medical and psychosocial aspects within the public health system.



4. We consider it essential to know the prevalence and incidence of Primary and Secondary Lymphedema and lipedema so that the necessary resources can be allocated to the treatment of these diseases. We want to promote epidemiological studies that address the health problems of people affected by lymphoedema and lipedema.



5. Research into Lymphoedema, Lipoedema, and Phlebedema, as well as all their possible treatments, should be promoted and supported for inclusion in National Health Plans.



6. We call for a review of some current practices that are used as treatments that pose a danger or deterioration to health, such as the indiscriminate prescription of certain drugs or the misuse of pressotherapy systems, among others.



7. We understand that age should not be a reason for exclusion in the assessment and treatment of the disease, as these are chronic pathologies, that can manifest at any stage of life.



8. We call for the establishment of programs for the early detection of Lymphedema, Lipedema, and Phlebedema, so that health professionals can refer the affected person to the corresponding services: Angiology and Vascular Surgery, Internal Medicine, Dermatology, Physical Medicine and Rehabilitation Service, etc.



9. There should be more information both for health professionals and for the patient, to be able to access the appropriate treatment as soon as possible, avoiding the worsening of the disease, which leads to an irreversible state and a greater degree of disability, which can have a negative impact on access to school, work, and social integration.



10. We request that the Ministry of Health and Science remove Manual Lymphatic Drainage from its list as a possible pseudotherapy that is under study, given that the General Council of Spanish Associations of Physiotherapists recognizes this technique as indispensable for the treatment of one of the most important sequelae of mastectomies, such as lymphedema. We also request that the Public Health System finance the maintenance therapies with manual lymphatic drainage appropriate to each patient, as it is currently being paid for by the patient.



11. We consider it necessary to include orthopedic garments appropriate to the needs of each patient in the portfolio of benefits of the Health Departments.



12. We ask that the limitations caused by Lymphoedema and Lipedema be taken into account when making assessments of Work Incapacity, Disability, and Dependency.



13. We want to be a valid interlocutor body with the Health Institutions in defense of the patient, establishing relations of training, information, coordination, and cooperation with the institutions and with similar national and international associations.

IMPORTANT NOTICE 

Lymphedema is a serious chronic and progressive disease due to an organic disability. The clinical treatment of lymphedema is neither a cosmetic nor an aesthetic treatment. The treatment of lymphedema is to control its progression and alleviate the symptoms related to dysfunction of the lymphatic circulatory system. To prevent and avoid serious complications associated with treatment, such as the possible displacement of edema to previously unaffected areas when compression therapy is applied for volume reduction, patients should use highly specialized and experienced therapists. Professional qualification and instruction delivered remotely online are not the same as live hands-on practical instruction in the clinical training and certification of lymphedema therapists, similar to all other rehabilitation, medical, and surgical training programs, and especially for developing the necessary manual skills to treat a disease as complex as lymphedema. 

Patients should also take special care when choosing a "Multidisciplinary Center of Reference for Lymphedema", as not all countries and centers provide the same treatment options. The best choice is a center of reference that provides “Complete Decongestive Therapy (CDT)”, which is recognized as the “Gold Standard” treatment for lymphedema.
