CUALES SON LOS DOS PRINCIPALES PROTOCOLOS DE TRATAMIENTO DE LINFEDEMA - CUAL ES LA DIFERENCIA ENTRE TERAPIA DESCONGESTIVA COMPLEJA (TDC) Y TERAPIA DESCONGESTIVA DEL LINFEDEMA (TDL) - Definición de Terminologías - Linfedema Primario y Pediátrico - Linfedema Secundario - Enfermedad Linfático-Vascular Orgánica - Guías de Mejor Práctica Clínica - Protocolo de Tratamiento - Medicina Preventiva - Estrategias de Rehabilitación - Vía de atención al paciente




A raíz de terminologías y siglas que están surgiendo en el campo de la medicina, es necesario tomar nota del nuevo término que se está utilizando ahora para designar el tratamiento "Low-Cost" para el linfedema.
 

Mientras que la Sociedad Internacional de Linfología (ISL) y la mayoría de sociedades científicas utilizan el término y sigla "Terapia Descongestiva Compleja o Completa (TDC)" para denominar el tratamiento conservador del linfedema, algunas organizaciones de salud y de linfedema están reemplazando y utilizando ahora el término modificado "Terapia Descongestiva del Linfedema (TDL)" para describir el tratamiento.


Hay que señalar que existe una gran diferencia entre los dos términos y opciones de tratamiento, ya que no son iguales y no son sustitutivas. Mientras que el término Terapia Descongestiva Compleja o Completa (TDC) se refieren al tratamiento que incluye todos los componentes que forman parte del tratamiento conservador, el nuevo término Terapia Descongestiva del Linfedema (TDL) se refiere a tratamiento en un sentido amplio y, sin la necesidad estricta de utilizar todos los componentes que forman parte del tratamiento conservador. El enfoque de tratamiento y el término designado como "Terapia Descongestiva del Linfedema (TDL)" se desarrolló como estrategia de ahorro con el fin primordial de limitar o excluir el Drenaje Linfático Manual (DLM) realizado por terapeutas a nivel de especialista.


Cuando se utiliza el término Terapia Descongestiva del Linfedema (TDL), este representa o designa muchas variaciones de tratamiento, incluso la opción de tratamiento formada por un solo componente para la fase inicial de reducción del edema, como puede ser el uso de bombas de presoterapia secuencial o el vendaje multicapa de forma aislada. El término "Terapia Descongestiva del Linfedema (TDL)" aparece pues como una opción de tratamiento alternativo de bajo coste para poder sustituir al tratamiento conservador.




¿QUE TÉRMINOS Y ABREVIATURAS UTILIZAN LAS SOCIEDADES CIENTÍFICAS PARA DESCRIBIR EL TRATAMIENTO DEL LINFEDEMA?

El término y sigla utilizados por la Sociedad Internacional de Linfología (ISL) y la Sociedad Científica Alemana (Guía S2K de Linfedema) son:

TERAPIA DESCONGESTIVA COMPLEJA (TDC)


¿QUÉ INCLUYE LA FASE INICIAL INTENSIVA DE LA TERAPIA DESCONGESTIVA COMPLEJA (TDC)? 

  • Cuidado de la Piel.
  • Ejercicios Descongestivos.
  • Drenaje Linfático Manual-DLM ( realizado por terapeutas a nivel de especialista).
  • Vendaje Multicapa (realizado por terapeutas a nivel de especialista).
  • Prendas de Compresión hechas a medida y de baja elasticidad (Tricotado Plano).
  • Educación del paciente.
  • Bomba de Presoterapia (es un adyuvante opcional y, sólo se aplica si va acompañado de Drenaje Linfático Manual (DLM) realizado por terapeutas a nivel de especialista).


Según la Guía de la Sociedad Científica Alemanano se recomienda la aplicación aislada de componentes en la Fase I Intensiva. La Terapia Descongestiva Compleja (TDC) debe utilizarse en su totalidad. 


La terapia de compresión aislada de las extremidades sin Drenaje Linfático Manual (DLM) puede aumentar la hinchazón en la raíz de la extremidad, e incluso puede provocar un Linfedema Genital.




¿QUE TÉRMINOS Y ABREVIATURAS NO UTILIZAN LA MAYORÍA DE SOCIEDADES CIENTÍFICAS PARA DESCRIBIR EL TRATAMIENTO DEL LINFEDEMA?

El término y sigla no utilizados por la Sociedad Internacional de Linfología (ISL) y la Sociedad Científica Alemana (Guía S2K de Linfedema) son:

TERAPIA DESCONGESTIVA DEL LINFEDEMA (TDL) 



¿QUÉ REPRESENTA LA NUEVA OPCIÓN DE TRATAMIENTO MODIFICADO DENOMINADA TERAPIA DESCONGESTIVA DEL LINFEDEMA
 (TDL) ?

1. Puede representar tratamiento consistente en el uso de un único componente para la fase inicial de reducción del edema, como puede ser la aplicación de bombas de presoterapia secuencial o el vendaje multicapa de forma aislada, sin la inclusión de Drenaje Linfático Manual (DLM) realizado por terapeutas a nivel de especialista. (Esto puede causar graves complicaciones para los pacientes).


2. Puede representar tratamiento para la fase inicial de reducción del edema, que suprima el vendaje multicapa realizado por un terapeuta a nivel de especialista y, sustituirlo por el auto-vendaje multicapa aplicado por el propio paciente. (Las sociedades científicas no lo recomiendan. No es lo mismo un vendaje para la reducción de un edema que un vendaje para el mantenimiento).


3. Puede representar tratamiento que suprime el Drenaje Linfático Manual (DLM) realizado por terapeutas para la fase inicial de reducción del edema y, sustituirlo por el auto-drenaje, una versión simplificada auto-aplicada por el propio paciente. (La eficacia de drenaje linfático simple o auto-drenaje como opción de tratamiento para la fase de reducción tiene poca evidencia científica).


4. Puede representar protocolos de tratamiento donde se indica el Drenaje Linfático Manual (DLM) realizado por un terapeuta especializado solo para el linfedema muy grave (Elefantiasis) o los casos complicados (cabeza, cuello y genitales), excluyendo así los linfedemas graves, moderados y leves. (Aquí no se contempla la Medicina Preventiva).






AVISO IMPORTANTE 

El Linfedema es una enfermedad crónica y progresiva debido a una disfunción linfática (Enfermedad Linfático-Vascular Orgánica). El tratamiento clínico del linfedema no es ni un tratamiento cosmético ni estético. El tratamiento del linfedema es para controlar su avance y aliviar los síntomas relacionados con la disfunción del sistema circulatorio linfático. 

Para prevenir y evitar problemas graves asociados al tratamiento, como el posible desplazamiento del edema a zonas previamente no afectadas cuando se aplica la terapia de compresión para la reducción del volumen, los pacientes deben recurrir a terapeutas altamente especializados con experiencia. 

La cualificación profesional y la instrucción impartida a distancia en línea no son lo mismo que la instrucción práctica en directo en la formación clínica y la certificación de los terapeutas del linfedema, similar a todos los demás programas de formación en rehabilitación, medicina y cirugía, y especialmente para desarrollar las habilidades manuales necesarias para tratar una enfermedad tan compleja como el linfedema.

Los pacientes deben igualmente tener un especial cuidado a la hora de elegir un "Centro Multidisciplinar de Referencia de Linfedema", ya que no todos los países o centros de referencia proporcionan las mismas opciones de tratamiento. La mejor elección es un centro de referencia que aplica la Terapia Descongestiva Compleja (TDC), que está reconocida como el tratamiento "Estándar de Excelencia" para el linfedema.

In the wake of terminologies and abbreviations that are emerging in the medical field, it is necessary to take note of the new term that is being used to designate the "Low-Cost" treatment for lymphedema


Whilst the International Society of Lymphology (ISL) and most scientific societies use the term and initialism "Complete Decongestive Therapy (CDT)" to designate the (Gold Standard) conservative treatment, some health and lymphedema organizations are now replacing and using the modified term Decongestive Lymphatic Therapy (DLT)" to describe the treatment.


It should be noted that there is a big difference between the two terms and treatment options, as they are not the same and they are not substitutive. Whilst the traditional term Complete Decongestive Therapy (CDT) refers to treatment that includes all the components that form part of conservative treatment, the new term Decongestive Lymphatic Therapy (DLT)refers to treatment in a broader sense and, without the strict need for the use of all the components that form part of conservative treatment.  The treatment focus and term designated as "Decongestive Lymphatic Therapy (DLT)" was developed with the primary purpose of limiting or excluding Manual Lymphatic Drainage (MLD) conducted by therapists at the specialist level.


When the term "Decongestive Lymphatic Therapy (DLT)" is usedit represents or designates many variations of treatment, including the treatment options made up of one single component, such as the use of compression pump devices (IPC) or multi-layer bandaging alone. The term Decongestive Lymphatic Therapy (DLT) appears thus as a "Low-Cost" alternative treatment option to replace the conservative treatment.




WHAT TERMS AND INITIALISMS ARE USED BY SCIENTIFIC SOCIETIES TO DESCRIBE LYMPHEDEMA TREATMENT?

The term and initialism used by the International Society of Lymphology (ISL) and the German Scientific Society (Lymphedema STK Guideline) are:

COMPLETE DECONGESTIVE THERAPY - (CDT)



WHAT DOES THE INTENSIVE PHASE I OF THE CONSERVATIVE TREATMENT COMPLETE DECONGESTIVE THERAPY - (CDT) INCLUDE?

  • Skincare.
  • Decongestive Exercises.
  • Manual Lymphatic Drainage (MLD) (conducted by therapists at the specialist level).
  • Multi-layered Bandage Wrapping (conducted by therapists at the specialist level)).
  • Custom-made low-stretch Compression Garments (Flat knit).
  • Patient Education (for the Maintenance Phase).

  • Compression Pumps (this is an optional adjuvant, and must be accompanied by Manual Lymph Drainage (MLD) conducted by therapists at the specialist level)).  


According to the German Scientific Society Guidelineisolated application of single components in Phase I is not recommended, and Complex Decongestive Therapy (CDT) should be used in its entirety. 


Isolated compression therapy of the extremities without Manual Lymphatic Drainage (MLD) may increase swelling at the root of the extremity and may even lead to Genital Lymphoedema.





WHAT TERMS AND INITIALISMS ARE NOT USED BY SCIENTIFIC SOCIETIES TO DESCRIBE LYMPHEDEMA TREATMENT?

The term and initialism not used by the International Society of Lymphology (ISL) and the German Scientific Society (Lymphedema STK Guideline) are:

DECONGESTIVE LYMPHATIC THERAPY (TDL)



WHAT DOES THE NEW MODIFIED TREATMENT OPTION DECONGESTIVE LYMPHATIC THERAPY (TDL) REPRESENT?


1.   It can represent treatment consisting of the use of one single component for Phase I (intensive or reduction phase), such as the use of pneumatic pumps (IPC) or multi-layer bandaging isolatedlywithout the inclusion of Manual lymphatic drainage (MLD) conducted by therapists at the specialist level(This can cause serious complications for patients).


2.    It can represent treatment for Phase 1(intensive or reduction phase) which suppresses multilayer bandaging conducted by a therapist at a specialist level, and replaced by patient-self-applied multi-layer bandaging. (This can cause severe complications for patients according to scientific societies. Bandaging for edema reduction is not the same as maintenance bandaging.).
 

3.    It can represent treatment that suppresses Manual Lymphatic Drainage (MLD) conducted by therapists at the specialist level for Phase 1(intensive or reduction phase), and replace it with self-drainage, a simplified version of drainage applied by the patient. (The efficacy of simple lymphatic drainage (SLD) as a treatment option for the reduction phase does not have sufficient scientific evidence).


4. It may represent treatment protocols where Manual Lymphatic Drainage (MLD) conducted by a specialized therapist is indicated only for very severe lymphedema (Elephantiasis Stage) and complicated cases (head, neck, and genitalia), excluding treatment for severe, moderate, and mild lymphedema. (Preventive Medicine is not considered here).






IMPORTANT NOTICE 

Lymphedema is a serious chronic and progressive disease due to lymphatic dysfunction (Organic Lymphatic Vascular Disease). The clinical treatment of lymphedema is neither a cosmetic nor an aesthetic treatment. The treatment of lymphedema is to control its progression and alleviate the symptoms related to dysfunction of the lymphatic circulatory system. 

To prevent and avoid serious complications associated with treatment, such as the possible displacement of edema to previously unaffected areas when compression therapy is applied for volume reduction, patients should use highly specialized and experienced therapists. 

Professional qualification and instruction delivered remotely online are not the same as live hands-on practical instruction in the clinical training and certification of lymphedema therapists, similar to all other rehabilitation, medical, and surgical training programs, and especially for developing the necessary manual skills to treat a disease as complex as lymphedema. 

Patients should also take special care when choosing a Multidisciplinary Expert Center of Reference for Lymphedema, as not all countries and centers provide the same treatment options. The best choice is a center of reference that provides Complete Decongestive Therapy - (CDT), which is recognized as the “Gold Standard” treatment for lymphedema.
