QUE ES FLEBOLINFEDEMA - Linfedema Primario y Pediátrico - Linfedema Secundario - Elefantiasis Linfostática



El Flebolinfedema es una condición singular de la combinación de insuficiencia circulatoria de dos sistemas, tanto el venoso como linfático. Hay una relación muy estrecha entre los sistemas venoso y linfático, por lo que una obstrucción venosa puede desarrollar una flebo - linfedema. Aquí nos encontramos con un fallo en el sistema de reflujo dual, y aunque son mecanismos hemodinámicas totalmente diferentes, son sistemas de circulación complementarios y mutuamente dependientes. Debido a sus funciones íntimamente integradas, el fallo de un sistema significa entonces la adicional carga y sobrecarga de la otro.


Hay varias causas: Flebolinfedema Primario y Flebolinfedema Secundario.

Flebolinfedema Primario es causado por un defecto congénito, con la implicación del comienzo de una difusa malformación linfática troncal, que afecta tanto a los sistemas venoso y linfático. Flebolinfedema primario es todavía una condición relativamente desconocida y en la mayoría de los casos representa la manifestación clínica del síndrome de Klippel-Trenaunay . La presencia de una malformación arteriovenosa hace que el estado general de flebolinfedema primario sea mucho más difícil de manejar. También se cree que una carga a largo plazo sobre el sistema linfático puede ser causa de un flebo-linfedema.


Flebolinfedema Secundario es una condición adquirida, que comienza con una insuficiencia crónica venosa debido a diversas causas, y que mas tarde conduce a la insuficiencia linfática crónica. En la presencia de hipertensión venosa, que es una característica de la mayoría de los trastornos venosos, el aumento en el flujo linfático se torna mucho mayor que la capacidad de transporte linfático. El tratamiento para el flebolinfedema secundario crónico, consiste en el tratamiento de la anomalía venosa y la observancia de la regresión del problema linfático.


El diagnóstico de flebolinfedema primario y secundario se basa en un historial clínico detallado, examen físico y pruebas relacionadas.


La mayoría de los pacientes con flebolinfedema necesitarán un curso de Terapia Descongestiva Compleja (TDC). La TDC consiste en una Fase Inicial para reducir el linfedema, seguida de una Fase de Mantenimiento de por vida para evitar la reaparición. Los tratamientos consisten en el Drenaje Linfático Manual (DLM), el cuidado de la piel, el ejercicio y el Vendaje Multicapa con vendas de compresión de baja elasticidad. Se recomienda que el tratamiento sea realizado por un fisioterapeuta de linfedema certificado.

El Linfedema es una enfermedad crónica y progresiva debido a disfunción linfática (Enfermedad Linfático-Vascular Orgánica)El tratamiento clínico del linfedema no es ni un tratamiento cosmético ni estético. El tratamiento del linfedema es para controlar su avance y aliviar los síntomas relacionados con la disfunción del sistema circulatorio linfático. 

Para prevenir y evitar problemas graves asociados al tratamiento, como el posible desplazamiento del edema a zonas previamente no afectadas cuando se aplica la terapia de compresión para la reducción del volumen, los pacientes deben recurrir a terapeutas altamente especializados con experiencia. 

La cualificación profesional y la instrucción impartida a distancia en línea no son lo mismo que la instrucción práctica en directo en la formación clínica y la certificación de los terapeutas del linfedema, similar a todos los demás programas de formación en rehabilitación, medicina y cirugía, y especialmente para desarrollar las habilidades manuales necesarias para tratar una enfermedad tan compleja como el linfedema.

Los pacientes deben igualmente tener un especial cuidado a la hora de elegir un Centro Multidisciplinar de Referencia de Linfedema, ya que no todos los países o centros de referencia proporcionan las mismas opciones de tratamiento. La mejor elección es un centro de referencia que aplica la Terapia Descongestiva Compleja (TDC), que está reconocida como el tratamiento "Estándar de Excelencia" para el linfedema.

WHAT IS PHLEBOLYMPHEDEMA - Pediatric and Primary Lymphedema - Secondary Lymphedema - Lymphostatic Elephantiasis



Phlebolymphedema is a unique condition resulting from the combination of circulatory insufficiency of two systems, both venous and lymphaticThere is a very close relationship between the venous and lymphatic systems, therefore a venous obstruction may develop a phlebo-lymphedema. Here we find a failure in the dual outflow system, and although they have totally different hemodynamic mechanisms, they are mutually dependent and complementary circulation systems. Due to their intimately integrated function, failure in one system means then additional burden and overload of the other.


There are various causes: Primary Phlebolymphedema and Secondary Phlebolymphedema

Primary Phlebolymphedema is caused by a congenital defect, involving an outset of diffused trancular lymphatic malformation, affecting both the venous and lymphatic systems. Primary phlebolymphedema is still a relatively unknown condition and in the majority of cases represents the clinical manifestation of Klippel–Trenaunay Syndrome. The presence of an arteriovenous malformation makes the overall condition of primary phlebolymphedema much harder to manage. It is also thought that Long-term strain on the lymphatic system can be a cause of phlebo-lymphedema.


Secondary Phlebolymphedema is an acquired condition that starts off with chronic venous insufficiency due to various causes, and which later leads to chronic lymphatic insufficiency. In the presence of venous hypertension, which is characteristic of most venous disorders, the increase in lymphatic flow becomes much greater than the lymph transport capacity. Treatment for chronic secondary phlebolymphedema consists of treating the venous abnormality and watching the regression of the lymphatic problem.


The diagnosis of primary and secondary phlebolymphedema is based on a detailed patient history and physical examination, and related tests.


The majority of patients with phlebolymphedema will require a course of Complete Decongestive Therapy (CDT). CDT consists of an acute phase to reduce lymphedema, followed by a lifelong maintenance phase to prevent a recurrence. Treatments consist of Manual Lymph Drainage (MLD), skin care, exercise, and Compression Bandaging with short-stretch compression bandages. It is recommended that treatment be performed by a certified lymphedema physiotherapist


IMPORTANT NOTICE 

Lymphedema is a serious chronic and progressive disease due to lymphatic dysfunction ( Organic Lymphatic Vascular Disease). The clinical treatment of lymphedema is neither a cosmetic nor an aesthetic treatment. The treatment of lymphedema is to control its progression and alleviate the symptoms related to dysfunction of the lymphatic circulatory system. 

To prevent and avoid serious complications associated with treatment, such as the possible displacement of edema to previously unaffected areas when compression therapy is applied for volume reduction, patients should use highly specialized and experienced therapists. 

Professional qualification and instruction delivered remotely online are not the same as live hands-on practical instruction in the clinical training and certification of lymphedema therapists, similar to all other rehabilitation, medical, and surgical training programs, and especially for developing the necessary manual skills to treat a disease as complex as lymphedema. 

Patients should also take special care when choosing a Multidisciplinary Expert Center of Reference for Lymphedema, as not all countries and centers provide the same treatment options. The best choice is a center of reference that provides Complete Decongestive Therapy (CDT), which is recognized as the “Gold Standard” treatment for lymphedema.
